Salary: $44.18 to 55.22 per hour
Hours: 37.5 hours per week; Variable shifts including on call

The Staff Pharmacist will be responsible for providing patient-focused care at the Toronto Rehabilitation Institute (TRI). Programs at TRI include spinal cord injury rehab at Lyndhurst Centre; complex continuing care, low tolerance long-term duration rehab, geriatric and dialysis rehab, and special care unit at Bickle Centre; neuro rehab, musculoskeletal and transplant rehab, and specialized dementia rehab at University Centre. Pharmacist responsibilities will include medication reconciliation; identifying, preventing, and resolving drug therapy problems; the development and maintenance of care plans; therapeutic drug monitoring; order verification; follow-up of medication incidents; responding to drug information questions; and patient and staff education. The pharmacist will work closely with other pharmacists, pharmacy technicians, and the interprofessional teams to ensure the provision of patient-centred care as well as the safe delivery and administration of medications to patients. In addition, the pharmacist will facilitate and support clinical teaching and research activities within the department, while exemplifying UHN’s values of safety, compassion, teamwork, integrity, and stewardship.
