The Calgary Foothills Primary Care Network (PCN) is a group of more than 600 family doctors and nurse practitioners who work with teams of other health professionals to offer patients the best in primary health care. Our PCN serves 400,000 patients in northwest Calgary and Cochrane. Together with our community partners, we offer round-the-clock care. We work to achieve better health outcomes for our patients. We have almost 200 staff (including health management nurses, clinical pharmacists, behavioural health consultants, social workers, dietitians and administrative staff) - and we are growing. The Clinical Pharmacist manages medications, conducts reviews, and provides evidence-based therapeutic information within a multidisciplinary primary care team. They also collaborate with patients to develop care plans, provide coaching for chronic disease management, and facilitate group-based health programs.
