The Clinical Pharmacist will provide comprehensive medication management by identifying and resolving drug therapy problems in collaboration with an interprofessional primary care team. They will also conduct patient assessments, provide education, and contribute to quality improvement and patient safety initiatives. Candidates must hold current registration as a Pharmacist with the College of Pharmacists of British Columbia and possess at least two years of recent, relevant patient care experience. A valid Class 5 driver's license and access to a personal vehicle are also required for the role.
