The PHARMACIST provides comprehensive pharmacy services to medical oncology/ radiation oncology inpatients within the PM Cancer Program, with cross coverage to the short stay and palliative care units. As a member of the patient care team, the principal responsibilities include: review and dispense all doctors’ medication orders for appropriateness; identify, resolve and prevent drug related problems with the patient and health care team; monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines; conduct medication reconciliation; patient, family and interdisciplinary team education; follow-up on medication incidents and promote safe medication practices on the unit; respond to drug information questions; provide direct and/or functional supervision of technician staff.
